Materials and methods: Twenty-one patients with anterior or distal displacement of the mandible treated in the Postgraduate Orthodontic Clinic of the Aristotle University of Thessaloniki had a digital lateral cephalometric radiography in fixed and in natural head position. Results: There was a significant difference in X-axis in the distribution (hence the mean results) according to the reliability of the landmarks and in only 2 variables in digital lateral cephalograms between fixed and natural head position. Conclusions: Cephalometric landmarks and variables showed reliability in digital lateral cephalo- metric radiography in fixed and natural head position.
